Sometimes an area does not exactly hurt, but it does not move well either. It feels tight, sensitive, guarded, or stubborn no matter how much you stretch it.

Instrument-Assisted Soft Tissue Mobilization, usually shortened to IASTM and often called scraping therapy, uses smooth-edged tools to apply controlled pressure and movement to muscles, tendons, fascia, and surrounding soft tissue.

At Hatch Sports Therapy in South Ogden, IASTM is used as one part of a larger bodywork session. It is not a treatment we automatically perform on everyone, and it is not a shortcut for “breaking up” tissue.

It is another tool for helping us work more precisely with areas that feel sensitive, restricted, or difficult to address with hands alone.

What Is IASTM?

IASTM uses specialized handheld tools that glide across the skin with varying amounts of pressure.

The tool gives both you and your therapist a different type of sensory input than traditional massage. This can be useful when working with areas that feel guarded, tender, stiff, or difficult to move comfortably.

Despite some of the mythology surrounding scraping therapy, the goal is not to aggressively break apart fascia or grind away scar tissue.

Bodies are a little more complicated than that.

A better way to think about IASTM is as a form of focused manual therapy. The combination of pressure, movement, sensation, and follow-up activity may temporarily change how an area feels and moves.

That window can make movement, stretching, strength work, or other hands-on techniques more productive.

Why We Use IASTM

IASTM can be especially useful when an area needs more focused input than broad massage pressure provides.

Clients often seek this type of work for:

  • Neck and shoulder tension

  • Upper-back stiffness

  • Hip and low-back tightness

  • Hamstring, quadriceps, and calf tension

  • Achilles and plantar fascia discomfort

  • Forearm and elbow tension

  • Areas affected by repetitive work or training

  • Persistent soft tissue sensitivity

  • Movement that feels restricted or guarded

  • Older injury or surgical areas that remain sensitive after appropriate healing

IASTM does not diagnose or cure an injury. It is one technique we can use when working with pain, tension, mobility, and movement problems.

IASTM for Runners, Lifters, Cyclists, Skiers, and Active Adults

Training has a way of finding your weak links.

Runners may notice stubborn calves or hamstrings. Cyclists often arrive with tight hips and quads. Lifters can develop persistent shoulder, forearm, or upper-back tension. Skiers and hikers tend to accumulate their own collection of complaints as the season goes on.

IASTM can provide targeted input to these areas without making the entire session about one technique.

That matters.

Your calf problem may involve more than your calf. Shoulder tension may change with work to the upper back, neck, rib cage, or movement patterns. A hip that feels tight may need mobility work rather than simply more pressure.

The technique should serve the session. The session should never serve the technique.

What Does Scraping Therapy Feel Like?

IASTM feels different from regular massage.

Depending on the area and amount of pressure, you may notice a scraping sensation, warmth, tenderness, or a strong pulling sensation across the skin.

It should remain tolerable.

Aggressive pressure is not the goal, and bruising is not proof that the treatment worked. Communication matters throughout the session, especially when treating an area that is already sensitive.

Some temporary redness or tenderness can occur afterward. Most mild skin reactions fade relatively quickly.

How IASTM Fits Into Your Session

At Hatch Sports Therapy, IASTM is rarely used by itself.

Depending on what we find during your session, it may be combined with sports massage, deep tissue work, assisted stretching, cupping, movement assessment, kinesiology taping, or simple exercises you can use between appointments.

We may also use movement immediately after IASTM.

That might mean moving the ankle after treating the calf, loading the shoulder after working around the upper back, or testing a movement that felt restricted before treatment.

This gives us useful information.

Did the area actually feel different? Did your range of motion change? Did a movement become easier?

Those answers matter more than how dramatic the treatment looked.

When IASTM May Not Be Appropriate

IASTM is not appropriate for every person or every area of the body.

Treatment may need to be avoided or modified with open wounds, active skin infections, fragile skin, significant bruising, uncontrolled swelling, recent trauma, blood-clotting concerns, or certain conditions that affect tissue healing.

If you recently had surgery, take blood-thinning medication, or are receiving medical treatment for the area, tell us before your session.

Sharp, worsening, unexplained, or newly traumatic pain may also need medical evaluation before bodywork.
